A user friendly and efficient way to communicate with Wellness Alliance. This free app allows patients, providers, staff, and/or community members to refer patients for a skilled home health evaluation by entering only a few details. Our Care Coordinators will promptly make contact to complete the process. Additional resources allow users direct access to skilled home health eligibility requirements and a description of the services we can offer the patients we serve.
